We would like to welcome you to the practice and address any health concerns you may have.
Please plan to arrive 15 to 20 minutes prior to your appointment. You will be asked to complete
new patient paperwork, including a health inventory. We will need to make a copy of your
health insurance card and your driver’s license or other picture ID.
Please make sure you bring a list of any medications you are on, including any herbal supplements.
Dr. West promotes wellness. Preventive care will be reviewed with you at this visit. Our goal is
to partner with you to pursue a healthy lifestyle.
To save you time in our office before your first appointment, you might want to print out and
