Policies


Appointments:
     Patients are seen by appointment only.  If it is necessary for you to cancel an appointment, we request 24 hours notice.  Please plan to arrive at the office 20 minutes before your appointment to complete your new patient paperwork.  You may download these forms in advance for your convenience.  You will be asked for your insurance card in order for the visit to be filed with your carrier.  A copy of your driver’s license will also be made.  To comply with insurance plan guidelines we will ask you to update your information on an annual basis.

Minors:
     Minor patients cannot be treated without written consent by the parent or guardian.  The co-pay, co-insurance or deductible is due at the time of service.  Parents should send funds with the patient or call the office to make charge arrangements through Visa or Mastercard.

Payment:
     Payment for services are due at the time of service.  If you are covered by an insurance plan in which  the office participates, we will be happy to file your claim with the carrier.  You are responsible for all co-pays, co-insurance, and deductibles at the time of service.  The office accepts cash, personal checks, Visa and MasterCard as payment.  If you do not have your insurance card at the time of service, you will be responsible for all charges.  Please make the office aware of any changes in your insurance prior to your visit.

Prescription Refills:
     Requests for refills should be made during regular office hours when we have access to your health history.  Your request may be made by phone or in person.  Prescription requests after  3:00 p.m. will be processed the next business day.
